"Win Judan/Gosei/Oza/Tengen twice" - what if somebody wins Oza and Tengen? Technically he did not win any of these tournaments twice (just one each). I guess that also count into promotion, however current text does not support it. Anybody knows?

In any combination. For example, Murakawa Daisuke was promoted to 9 dan by winning Oza and Judan.
