Fujitsu 1993 Teire Incident

According to Go World 71, page 37, following happened in the first round of the 6th Fujitsu Cup, 3 April 1993:
Takemiya Masaki, White, played Yang Hui. Move 245 was the last endgame move, after which neutral points were filled alternately. As Takemiya failed to connect his stones (see teire), Yang cut (at P18). However, since Japanese rules were in effect and both knew that Yang had won anyway (B+0.5), they agreed to let White connect.
